Therapie eines „Buried Bumpers“ mittels NOTES – ein Fallbericht

Zusammenfassung

Beim „Buried-Bumper-Syndrom“ kommt es zu einer Migration der inneren perkutanen endoskopischen Gastrostomie (PEG) Halteplatte in bzw. durch die Magenwand. Therapeutisch bleibt in schweren Fällen, z. B. bei Migration der Halteplatte in die freie Bauchhöhle mit konsekutiver Peritonitis, häufig nur eine chirurgisch-laparaskopische Entfernung. In unserem Fallbericht beschreiben wir die erfolgreiche Bergung einer intraabdominell migrierten Halteplatte mit anschließender PEG-Neuanlage mittels transgastraler Peritoneoskopie im Sinne von NOTES (Natural Orifice Translumenal Endoscopic Surgery).

Abstract

Mucosal overgrowth and migration of the inner bumper into or even through the gastric wall (= buried bumper syndrome) is a possible complication of percutaneous endoscopic gastrostomy (PEG). In this case report we describe the successful endoscopic recovery of an intraabdominally migrated inner bumper as well as reestablishment of PEG using NOTES (Natural Orifice Translumenal Endoscopic Surgery). This practical application of the NOTES technique via the preexisting transgastric approach (PEG) can therefore be a reasonable alternative to a surgical-laparoscopic therapy of the severe buried bumper.
